The study investigated the effects of matrine on Wilms tumour (WT) cells and evaluated the clinical outcomes of matrine injection combined with preoperative transcatheter arterial chemoembolisation (TACE) in children with WT. Primary WT cells were treated with matrine (0 - 1.5 mg/mL), and cell proliferation and apoptosis were assessed using MTT assays and flow cytometry. Additionally, 65 children with WT received either TACE alone (n = 33) or TACE combined with matrine injection (n = 32). Clinical outcomes, serum tumour markers, tumour-free survival (TFS) and adverse reactions were evaluated. Matrine inhibited WT cell proliferation and increased apoptosis in a concentration-dependent manner (p < 0.05). Compared with TACE alone, combined treatment was associated with shorter hospitalisation, faster renal function recovery, higher complete tumour resection rates, and reduced serum tumour marker levels (p < 0.05). TFS rates were 87.9% and 93.8% in the TACE and combination groups, respectively, while adverse reactions were less frequent with combined treatment. These findings suggest that matrine may enhance the therapeutic outcomes of TACE in children with WT.
