Abstract:

The goal of this retrospective study would be to trace if the clinical physicians’ recommended medications complied with the newest guidelines for hypertension treatment. Likewise, we tried to identify the pharmacist’s role in patients with hypertension health care plan, as a member of the medical team.
The eligible patients were selected among those hospitalized in the internal medicine hospital. Their records were summarized and some facts were registered: demographic data, diagnosis, blood pressure values at reception, other significant clinical and lab tests results, associated diseases and the prescribed therapy.
Results revealed a high incidence of major cardiovascular events. The goals of the previous antihypertensive therapy were not achieved, except for a small number of patients (16%). In some cases, the actual guidelines recommendations were ignored. Thus, some ways were identified for the pharmacist to involve himself and the physician, together to ensure a well-advised treatment, by preventing and solving problems coming along with medication and by increasing patient’s compliance.
