AGGREGOMETRIC MODIFICATIONS TO THIENOPYRIDINES ANTIPLATELETS DRUGS USED IN CORONARY ARTERY DISEASE.

Abstract:

Adenozine diphosfate (ADP) plays a critical role in modulating thrombosis and haemostasis. ADP initiates platelet aggregation by simultaneous activation of two receptors: P2Y1 and P2Y12. Activation of P2Y1 activates phospholipase C and triggers shape change, while receptor P2Y12 couples to Gi to reduce adenylyl cyclase activity.
The receptor P2Y12 has been shown to be the target of the thienopyridine drugs, clopidogrel and ticlopidine.
The purpose of this study was to show the modified aggregometric response due to thienopyridines antiplatelet therapy (clopidogrel and ticlopidine) in 30 patients with coronary artery disease (instable angina and acute myocardial infarction, angioplasty and coronary stent) hospitalized in Clinical Institute Fundeni. The doses administered to patients were 75mg/day for clopidogrel and 250mg x 2/day for ticlopidine. The tests were started after 5-7 days from 1st administration, time interval necessary for platelet inhibitor effect to be obvious.
The platelet aggregation was done in vitro, using the aggregometer Chrono-Log and lumi-agregometry method. The blood samples drawn from patients were specifically prepared in order to obtain platelet rich plasma. Platelets from platelets rich plasma were stimulated with various aggregating agents: ADP, epinephrine, collagen.
The results obtained have shown that platelets from the blood drawn had a hipoagregability response to ADP and also to the other aggregation agonists used.
